What level do the crystals spawn?

I think on all levels? The alter is on level one and I had a vampire bat drop a crystal.

Hythloth does spawn a boss. Boss is harder than fafnir. All levels spawn crystals accept the grinch dungeon (please page eppy to add them there- they are even used as Deco on that level already).
